THE AUTHOR'S OWN COPY IN PERSONALISED BINDING WITH HIS SIGNED INSCRIPTION ON FRONT FREE ENDPAPER. The author was Honorary Dental Surgeon at London's Royal Dental Hospital and several other institutions.
